Inhabitants are still tethered to Amani (ex. they still can't get pregnant outside of the simulation because of their connection to it) so, yes, event effects can persist even if people leave! People can transform, experience negative side effects, and so on. In the case of Hiromitsu, the yokai forms are 'default' for safety reasons - yokai tend to eat mortals and other species - but can be toggled off if one wants. Just don't say they weren't warned! Similarly, they can influence worlds outside of Amani via intimacy just not at quite an extreme level.
If you need a physical reason why this is, each inhabitant has a microchip implanted between their C1 and C2 spinal vertebrae that would be fatal it attempted to remove. Most won't even know it's there unless pointed out, but it serves as a way to track them as well as keep their Amani abilities.
